Team Roster

The captain or coach of each team should download the team roster form here. This form must be turned in with all team player names and t-shirt sizes prior to first game. No one can participate in the tournament unless their name is on the roster.

Softball Waiver Forms

The captain or coach of each team should download this waiver form for each of their team members to sign and return to the tournament organizer prior to the first game. No player can participate in the tournament without having this waiver signed and turned in.
