If you're a first time sponsor and your donation is a triple level or higher please provide the following below.  If you are a returning sponsor but your donation is a first time triple level or higher, please provide the following below. 


If you don't have all of your artwork or logo now, no worries, you can send it at a later time via email. We would like to have as much artwork up front if possible.  All artwork must be received by June 1st.
